The Integrated Medicine and Rehab Services (IMRS) program at Orillia Soldiers’ Memorial Hospital (OSMH) is a redesign of care for patients that will focus on an inter professional model of wraparound care, enhancing the transitions between phases of care and the continuity of care provided by inpatient, outpatient and community teams working together. Patients admitted to this unit will be able to receive acute medical care, rehab care, palliation without ever having to change rooms or care teams.
Our IMRS unit is located on the fourth floor of the Community Tower and Harvie Wing. Care is provided by our team of geriatrician, registered practical nurses, registered nurses, dietitians, occupational therapists and physiotherapists, recreation therapist, clergy, pharmacist, and volunteers, who play a large role in helping patients reach their goals. Families are encouraged to take an active part in patient care.
